My Recordkeeping Workshop Forms are Now Available on a CD that Allows You to Complete & Print them with the free program  Acrobat Reader

After receiving many requests from individual clinicians who have attended my recordkeeping workshop over the past few years, I have transformed my paper-based clinical forms to an electronic format that allows them to be completed and printed via the free online program known as Acrobat Reader, and YOUR computer! (MAC people--- you know why you are! See below!)

The electronic versions of the forms (which are always provided in paper format to all recordkeeping workshop attendees), are now available for purchase on a CD, that when opened with the free online program known as Acrobat Reader, can be completed and printed with your computer printer. [PC people--- You might even find a computer guru who can tell you how you can save a copy of the completed forms!]

The Details:

The forms on the CD are pretty easy to use.  If you haven’t already, you must first download and install the free online program known as Acrobat Reader. [I recommend the newest version 8.1.1 due to its capability of highlighting "fillable" areas of the forms--- at least on PCs that's what happens!]

Then place your new psychotherapy e-forms disk in your computer CD-drive compartment.

Open Acrobat Reader, first click on "file" and then "open" (that appears in the drop-down list under "file"). In the "Open" window, locate your CD drive, double click that icon, and you should then see a list of the folders and some individual files that are on the CD. I recommend you begin by opening and reading the file titled, "Open Me First".

I encourage you to explore the folders and files that are on the CD. When you open a form-file with this new version of Acrobat Reader, you should see a bright pink bar across the top of the form, and on the right of this pink bar are the words, "highlight fields". I suggest that when working with the forms you click on that to keep the "fillable" fields highlighted, as it makes it very clear where you can fill in the form with typed information.  Please note the printer icon on the upper left of the screen— this is the "print" button. Acrobat Reader does not allow you to save a completed form, so be certain to print one for your records.

Special Message for MAC Computer Owners:

I am and always have been a PC user and personally
don’t know much about Mac Computers.  I know, I know, I hear they are wonderful...

I asked a good friend/colleague / former PC person who went
over to the "dark side" (i.e., they bought a MAC and they love it),
to test out these files on their new MAC.

This was their response:   "I can open the forms and fill them in, using "tool mode".
I don’t think my Mac requires Adobe Reader, as it switches it into "Preview"
which is a Mac function. The areas of "fill" are not blue, it’s just a form.
I can fill in a few slots and e-mail it back to you."   Which, by the way, she did—
and what this means is that her Mac was able to "save" the filled-in file—
something I cannot get my PC to do with Adobe Acrobat!  Rats!
